Each player at Phoenix must have a signed waiver. Any player under the age of 18 must have a parent or guardian signature.
Please print waiver, have it filled out and signed when you come to play so we can get you into the game faster.
Please note: We are not able to allow anyone under the ago of 10 to play at PHX due to liability concerns.
